Precision Health Systems LLC is a healthcare operations and telehealth organization supporting patients, home care agencies, care coordinators, rehabilitation facilities, and community-based organizations throughout New York.

We are seeking a reliable and patient-centered Nurse Practitioner to join our growing telehealth team. This is an excellent opportunity for an NP looking for a flexible part-time schedule while providing meaningful care to patients who may have difficulty accessing traditional office-based services.

The NP will work under the clinical oversight of Dr. Nonkulie Dladla and will collaborate with our operations team to provide timely patient evaluations and appropriate follow-up care.

Position Responsibilities

The NP will primarily provide remote telehealth evaluations and follow-up visits, including:

  • Conduct comprehensive patient evaluations via secure telehealth platform
  • Evaluate patients referred for home care and community-based services
  • Assess medical conditions, functional limitations, and patient care needs
  • Review medications and relevant medical history
  • Evaluate homebound status and functional limitations when appropriate
  • Determine medical necessity for requested services and equipment
  • Complete required clinical documentation and forms
  • Provide appropriate treatment recommendations and referrals
  • Coordinate with PCPs, home care agencies, care coordinators, and other healthcare professionals
  • Provide patient and caregiver education
  • Identify when an in-person evaluation or additional medical testing is necessary
  • Maintain timely and accurate documentation within the EMR
  • Communicate professionally with patients, caregivers, and referral partners

Depending on patient needs and referral requirements, evaluations may involve:

  • Home care evaluations
  • NYIA-related evaluations
  • M11Q documentation
  • CHHA/home health evaluations
  • Custodial care assessments
  • Medical Necessity Letters
  • DME evaluations and documentation
  • Incontinence supply requests
  • Therapy referrals
  • Skilled Nursing evaluations
  • Transportation-related medical documentation
  • Chronic disease management
  • Follow-up telehealth visits
  • CCM/RPM patient management as the program expands

Schedule

3-5 days per week

We will work closely with the NP to coordinate appointments efficiently. Our goal is to group appointments into consistent time blocks rather than scheduling sporadic appointments throughout the entire day.

This allows the provider to maintain a predictable schedule while still providing timely access to patients.

Position Requirements

Required:

  • Active New York State Nurse Practitioner license
  • Current NP certification
  • Ability to provide care through telehealth
  • Strong clinical assessment and documentation skills
  • Reliable internet connection and appropriate private workspace
  • Ability to communicate effectively with patients and caregivers
  • Comfortable working independently within a structured clinical workflow
  • Current DEA registration, if applicable to prescribing responsibilities

Preferred:

  • Primary care/Internal Medicine experience
  • Telehealth experience
  • Experience with home care or community-based services
  • Experience working with Medicare/Medicaid populations
  • Familiarity with NY home care documentation
  • Experience with chronic disease management
